Diabetes Management Program

The Glenrock Country Practice Diabetes Program aims to provide and maintain a high quality and systematic approach to diabetes management for patients of our practice.

The main objectives of the Diabetes program are to:

  • Improve the management of patients with diabetes
  • Provide efficient and effective diabetes management in the general practice setting
  • Implement the Best Practice Guidelines for the Management of Diabetes Mellitus
  • Improve the capacity of patients with diabetes to better manage their condition

The  Practice-based diabetic program may involve diabetetic educator, nurse, dietician, podiatrist, psychologist, exercise physiologist and your pharmacist.  The team conducts a comprehensive diabetes assessment for each patient, as well as providing one-on-one patient support and guidance – in collaboration with the GP. Recent results from blood tests are regularly used to review and assess the current level of diabetes management. 

Patients are provided with additional attention, time, and education material to motivate and encourage the patient to self-manage their condition effectively.

The Diabetes program adheres to evidence-based best practice management guidelines so better health outcomes are assured.
